Fortunately, the rare drink is readily available and not too pricey from The Main Ingredient, an alchemy shop in the Market District of the Imperial City, for about 130 gold.
After the purchase of the Brandy, return to the shrine and place it on the altar. Having made the offering, the air distorts, and you can nearly hear the sound of laughter in your head from a thousand invisible revelers. Then, out of thin air between worlds, Sanguine himself appears, boisterous, mischievous, and filled with energy. He greets you with a chuckling voice, complimenting your sense of humor and sense of enjoyment before setting you to work on a task befitting his personality: "There's a Countess in Leyawiin who's lost the ability to laugh. Let's remind her." And so you set off on your downhill slide into decadent corruption.
Gaining Access to the Countess's Party
Sanguine's quest takes you to Castle Leyawiin, the home of Countess Alessia Caro. The Countess is renowned for practicing strict formality and propriety, making her the perfect victim for Sanguine's game of lighthearted humiliation. To begin, travel to Leyawiin and approach the castle in the evening. The Countess hosts a formal dinner party between 6 PM and midnight, and you’ll need to gain entry to participate. There are two main approaches, one focused on fashion, the other on persuasion. The simplest method is to purchase the Blue Velvet Outfit, which is a pleasant piece of attire that can be purchased or stolen from Divine Elegance in Imperial City's Market District. Equip it, and remove any armor, weapon, or shields; appearances matter here at this soiree. You may still wear rings or amulets, as jewelry is not frowned upon. Dressed to your best, approach the castle gate guard and speak with him. Overwhelmed by your attire, he will open the gate for you without a second thought. Or, if you like the charm approach, you can talk to the same guard between 6 PM and persuade his Disposition up to 70 or more with the Persuasion Wheel or with some carefully placed bribes. As soon as his attitude is softened, he will smile and wave you in, assuring you that the Countess loves having sophisticated guests. Either way, after you reach the great hall, save your game. The second half is where the real pandemonium begins.
The Stark Reality Spell - Sanguine's Gift of Chaos

As you mingle with the nobles, you will see Countess Alessia Caro, seated at the head of the long feast table. She wears a beautiful green dress and carries an air of haughtiness that would even cause Daedra to blush. Now, go see what's on your Magic menu. Sanguine has passed along to you a one-of-a-kind spell called Stark Reality, a horrible area-of-effect illusion spell that doesn't appear anywhere else in the game. Activate it, target the Countess, and blast. In the time it takes to blink, the entire banquet bursts into laughter, pandemonium, and scandal. The Stark Reality spell strips everyone of clothing, down to their underwear, and all objects in your inventory, including armor, gold, and weapons. You're left standing nearly naked in front of the stunned nobles. The results are immediate and chaotic. Guests scream, guards draw weapons, and the Countess herself gasps in fury. While the spell technically only causes Drain Health 1 Point on Target, this tiny effect counts as an assault, meaning the guards will come rushing to arrest you. You’ve completed your part of the prank, but now comes the tricky part: surviving the aftermath.
Escaping or Submitting to the Guards
When you cast Stark Reality, you have two options: surrender or flee. They both complete the quest, but how you get there affects the short-term experience. When you try to flee, run for the castle gate and proceed directly right as soon as you're outside. The Leyawiin guards won't be so generous, however, and since your equipment is gone, you can't fight or bribe them. You're likely to be trapped or murdered unless you move fast and can outwit them in the wilderness. The more practical (and often recommended) route is to surrender peacefully. When the guards catch you, choose to go to jail. You’ll spend a short stint behind bars, losing one point in a few random skills, but it’s a small price to pay for successfully pulling off Sanguine’s prank. While in jail, you can just sleep in the bed they offer to pass the time. Upon release, your inventory will be stripped, but don't worry, Sanguine has taken care of that.
Return to Sanguine's Shrine
Once you've escaped the ire of the Countess (and probably the Leyawiin penal system), return to the Shrine of Sanguine to report your victory to the Daedric Prince. The Daedric Prince will greet you with thunderous rolling laughter, clearly delighted by the devastation you wrought. "A masterpiece! You've given them an episode they'll never forget. Take this, and remember me when you want to spice up the banality of mortal life." And so, he rewards you with his Daedric artifact: the Sanguine Rose.
The Power of the Sanguine Rose

The Sanguine Rose Oblivion is one of the most interesting and crazy weapons in the game. It appears as a tall, dark staff with a red flower at the end, lightly pulsing with Daedric energy. When in hand, it summons a random Daedra to fight for you. Every time you use it, a random creature is released, occasionally a Scamp, sometimes a Dremora Lord or Clannfear. The Daedra summoned will go after the target you aimed the Rose at, tearing through foes with maniacal glee. Once their target is killed, though, they attack you. Thankfully, the Daedra are persistently despawning after roughly 20 seconds, vanishing in a puff of smoke and euphoria. The Sanguine Rose does not drain the player's Magicka but instead relies on a set of charges, which can be replenished by Soul Gems. It is perfect for players who want to introduce a bit of chaotic humor into battle, or anyone who wants to carry around one of Tamriel's most dreaded artifacts.
